What are you looking for?

Everyone seeking to receive services for people with developmental disability from the Illinois Department of Human Services either immediately or in the future must signify such intent by registering in the PUNS database.

This database serves as the waiting list for service seekers. CSO conducts a face-to-face meeting with everyone seeking services and assists with completing PUNS registration. Once the individual’s name, demographic information, type of services being sought and the time frame by which the services are needed are entered into PUNS, the information are transmitted to DHS.

At its sole discretion, DHS conducts a selection process. Individuals selected are then notified to contact their ISC agencies for further processing. To preview DHS’s PUNS brochure, click here. To sign up for PUNS, call our Intake department at (773) 471-4740.
